SDS Workers who are C.N.A.s
This memo is for you!
In order to keep you C.N.A. certification valid you will also need to work for pay at least 8 hours during the previous 24 months under the supervision of a Registered Nurse at a licensed home health agency, nursing home, hospice or hospital. If you only work as an employee for your Self Directed Support (SDS) client, this WILL NOT qualify you to remain on the C.N.A. registry. Some of you have attempted to be recertified with “HEADMASTER” and found that you have been denied recertification. The reason why you were denied is that the SDS program is not Medicare or Medicaid certified approved program for C.N.A.s. Please remember to apply to work at one of the agencies listed above if you intend to keep your hard earned C.N.A. certification active. Once lost you will have to reestablish federal employment on the Wisconsin Nurse’s Aide Registry (WNAR) and must successfully pass both components of the approved Wisconsin nurse aide competency examination.

Checklist for Employees
Indicate the times you worked each day (e.g. 8:30 a.m. - 10:30 a.m.). Then, total the amount of hours worked that day.
Total the number of hours worked under each pay period prior to having your member sign your time sheet.
Submit your time sheet by the due date and time. Employees can submit their time sheets via email, fax, in person, or by mail.
Late time sheets will be paid in the following pay period.
