When To Notify Our Office

  • If either the member or employee have a change in address, contact information, or marital status.
  • If the employee’s direct deposit information changes, it must be submitted at least ten days prior to the pay date.

  • If the employee would like to change federal or state tax elections.

  • If the member has a change in status, such as going to the hospital, going out of town, or an increase or decrease in SDS services.

  • If the member is leaving the My Choice Family Care program, please notify us of date and reason.

  • If a work injury has occurred.

  • If an employee needs a payroll summary for state benefits, loans, or verification. Please provide two business days for the report. A $5 fee will be collected from the employee for immediate requests.

  • Remember our office hours are Monday to Friday, 7:30 a.m. to 4:00 p.m.
