Worker’s Compensation

Employees are covered by a group Worker’s Compensation policy. If an employee is injured while working, the member and employee must complete a worker injury form within 24-48 hours of the incident. This form can be found on the SDS Payroll Services website. To complete this form you will need the employee’s work schedule and duty sheet of services provided.
